What this is

A reference desk: dated lists and field guides, written so a reader can check the facts themselves. Entries are shorter than essays and longer than a bare directory listing.

How entries are researched

Every entry names its sources in a closing section: provider documentation, hospital directories, insurer panel documents, public pricing pages. Where a number is an estimate, the text says so.

How often entries change

Hospital scope, cloud pricing and compliance requirements move. Entries carry a publish date and are revised when the underlying source changes, not on a fixed schedule.

Corrections

If an entry is wrong, the correction is applied to the entry and noted in the revision date. There is no separate errata page.
